Complete this entry form to participate in the 18th Annual Exhibit of The National Arts Program for UChicago Medicine employees and their family members. There is no fee to enter the program.

    Eligibility

    • Participants must be an employee, retiree or immediate family member of an employee of UChicago Medicine, an employee or retiree of BSD or an employee or retiree of the Pritzker School of Medicine. 
    • Only 2 entries per artist will be admitted.
    • All entries submitted must be the original work of the applicant completed within the last three years and not previously submitted.
    • The National Arts Program and UChicago Medicine reserve the right to disqualify any entries they consider to be unacceptable or inappropriate for a public exhibit.

    Entries
    • Submit only entries which fit into the listed medium categories.
    • All artwork will need a NAP identification label placed on the back (provided at drop off).
    • All 2-dimensional entries should be matted, framed and/or ready to hang with wire securely on the back.
    • Only 3-dimensional entries that are ready and able to be hung and measure no more than 4” in depth will be accepted.
    • All work must measure no more than 48” in any direction or weigh more than 50lbs unless otherwise stipulated.
    • Artwork that does not meet size limitations or specifications will be disqualified before judging.

    • Deadline This Registration Form must be received no later than October 28, 2024.
    • Delivering Entries All entries must be delivered to Main Lobby of Mitchell Hospital from October 29 - 31 between 9am - 4:30pm.
    • Reception & Awards Details regarding the awards reception will be sent out to participants via email as soon as it becomes available. 
      • Prizes will be awarded by classification:
        • Adult (Amateur, Intermediate and Professional): 3 - First Place Awards of $350; 3 - Second Place Awards of $250; and 3 - Third Place Awards of $150
        • Youth / Teen : 2 - First Place Awards of $150; 2- Second Place Awards of $100; and 2 - Third Place Awards of $50
        • A Best of Show Award of $400
        • An Art Education Award of $200
      • Honorable Mention Ribbons will be awarded at the discretion of the judges.
      • Awards are limited to one cash award per artist.
    • Public Exhibit The exhibit is free and will be open to the public from November 5, 2024 - May 1, 2025.
    • Retrieving Entries Each artist is responsible for retrieving their artwork from Skybridge on Thursday, May 1, 2025 between the hours of 9am - 4:30pm.  No work is to be removed prior to the end of the exhibition. Neither UChicago Medicine nor the National Arts Program will be responsible for unclaimed works.
    • Liability Every attempt will be made to preserve, secure and protect each entry submitted; but neither the National Arts Program nor UChicago Medicine can be held responsible for damages, loss of art entries, or changes/cancellation of this exhibit due to low participation or unforeseen circumstances.  The National Arts Program and UChicago Medicine reserve the right to disqualify any entries they consider to be unacceptable or inappropriate for a public exhibit. The placing of art in the exhibition constitutes an agreement on the part of the entrant with conditions herein set forth.  The National Arts Program and UChicago Medicine reserve the right to publish for publicity purposes reproductions of the entries.  The National Arts Program at UChicago Medicine, is a public exhibit and photographing/filming of the exhibit is encouraged.
