Complete this entry form to participate in the 11th Annual Exhibit of The National Arts Program for Aurora St. Luke’s Medical Center employees, retirees, volunteers, and their family members. There is no fee to enter the program.

Your registration is not complete until you have clicked the ‘submit registration’ button and receive an automated confirmation email.

Eligibility

  • Participants must be an employee, retiree, volunteer, medical resident or an immediate family member of an employee, retiree or volunteer of Aurora St. Luke’s Medical Center.
  • Only 2 entries per artist will be admitted.
  • All entries submitted must be the original work of the applicant completed within the last three years and not previously submitted.
  • The National Arts Program and Aurora St. Luke’s Medical Center reserve the right to disqualify any entries they consider to be unacceptable or inappropriate for a public exhibit.

Entries
  • Entrants may submit only two entries that fit into the listed medium categories.
  • All artwork will need an NAP identification label placed on the back (provided at drop off). 
  • All 2-dimensional entries should be matted, framed and/or ready to hang with wire securely across the back. 
  • All work must measure no more than 48” in any direction or weigh more than 50 pounds unless otherwise stipulated. 
  • Artwork that does not meet size limitations or specifications will be disqualified before judging.

Rules of Exhibit
  • Deadline This Registration Form must be received no later than March 3, 2025 at 5:00pm to participate in this exhibit.
  • Delivering Entries  All entries must be delivered on Thursday, March 6 between 8am - 8pm at Aurora St. Luke's Medical Center Art Therapy Office, H6361 Center Building 6th Floor.
  • Reception & Awards A reception and presentation of awards will be held at the Aurora St. Luke’s Medical Center Skywalk on Thursday, April 24, 2025 from 5:30pm - 7pm.
    • Prizes will be awarded by classification:
      • Adult (Amateur, Intermediate and Professional): 3 - First Place Awards of $350; 3 - Second Place Awards of $250; and 3 - Third Place Awards of $150
      • Best of Show Award of $400 
      • Youth / Teen : 2 - First Place Awards of $150; 2- Second Place Awards of $100; and 2 - Third Place Awards of $50
      • Art Education Award of $200
    • Additional awards including Honorable Mentions will be awarded at the discretion of the judges.
    • Awards are limited to one cash award per artist per classification.
  • Public Exhibit The exhibit is free and will be open to the public from March 24 - May 29, 2025.

  • Retrieving Entries Each artist is responsible for retrieving their artwork from the Aurora St. Luke’s Medical Center Skywalk, 3rd floor main building on May 29, 2025, between 8am - 8pm.  No work is to be removed prior to the end of the exhibition. Neither the Aurora St. Luke’s Medical Center nor the National Arts Program will be responsible for unclaimed works.

  • Liability Every attempt will be made to preserve, secure and protect each entry submitted; but neither the National Arts Program nor Aurora St. Luke’s Medical Center can be held responsible for damages, loss of art entries, or changes/cancellation of this exhibit due to low participation or unforeseen circumstances. The National Arts Program and Aurora St. Luke’s Medical Center reserve the right to disqualify any entries they consider to be unacceptable or inappropriate for a public exhibit. The placing of art in the exhibition constitutes an agreement on the part of the entrant with conditions herein set forth.  The National Arts Program and Aurora St. Luke’s Medical Center reserve the right to publish for publicity purposes reproductions of the entries.  The National Arts Program at Aurora St. Luke’s Medical Center, WI is a public exhibit and photographing/filming of the exhibit is encouraged.
